if its from skin pulling on the cage as the erection stiffens, use lotion or some form of lube on the base ring before bed. this will allow it to slide and not wake you with a pinching sensation.
the body will get used to the cage being there. it takes 1-3 weeks from my experiences.

When i first started to wear a cage it was a cb-2000 and I used to be woken up in the night after only the briefest time asleep. The sensation was as though some invisible hand was attempting to rip out my kidneys by pulling on my balls.

It was the wrong cage for me.

Through a combination of trial and error with different cages ( with a wide range of price tags) and becoming accustomed to full time wearing I eventually found myself not in too much distress and then only rarely.

What I have subsequently discovered works best for me and only in the last couple of years is that if the cage diameter wise is;
a snug enough fit that when flaccid I have room for my skin to breathe and to be able to rinse thoroughly between washes
whilst at the same time it makes attempts to become erect are no more than attempts.

This set up for me produces no physical pain.

Of course this also means that along with orgasm control your KH has also got erection control, my Lady enjoys both aspects prety much equally, with the added bonus that the feelings of my needing an erection return much more quickly and frequently with such intensiy that the feelings that I need to cum have become secondary.

i feel i can still get an erection it just kinks it and angles downward ever so slightly.
it wont allow a full erection i suppose, and certainly not one that feels good. more of a stifled smashed one that keeps me from even thinking of enjoying it.
fun none the less. anyone with a PA ever have erections tug on the skin? i sometimes find that after a huge erection and then it goes flacid, the skin wont slide back into the cage leaving the penis head away from the end of the cage

I was using a tether, however I ordered another device with 1/8 increase in gap and that made all the difference in the world! No more tether and no more waking up in the middle of the night or early morning!

 

Like others I suggest embracing it. Wake up. Sit down to pee and release the pressure and remind yourself where you are in life. Just don’t wake your KH.

 

yes this works sometimes.
at the worst, i was being woken every 15 minutes. sometimes cage adjustments need to be made.
for me opening the base ring as well as increasing the gap between the cage and the sheath was crucial. i also angled the cage slightly less downward facing, which made urinating a ton easier as well. my cage is freaking perfect right now. very glad i stuck it out though as there were days when i was ready to say forget this, 24/7 isnt possible.

 

to avoid your problems
you need to mix two solutions :
a small small CB and wearing it each night
after about 2 weeks, you'll sleep along
but only one night without and you've to return to the beginning

 

As others have said, it does get better with time. For me there are mostly good nights now. On the bad nights it really turns my wife on when I wake her up moaning from the pain. When she wakes from my moaning she'll check my cock for circulation and when she sees there's no problems she'll tease me and make it worse. Then she'll go pee, check my cock again and go back to sleep.

 

Oval base rings is the only thing I have found to cure that issue for me. Round rings wake me up with intense pain, but the oval in the same diameter and I sleep through the night.

 

My first night in a properish fitting ball trap device. Very comfortable while flacid. No testicle pull out yet. Kinda rough when I get erect. The ring pulls on my testicles when I get erect. I woke up twice last night. 2am I was able to get things to settle down. 5am I tapped out and took it off.
