What you describe is pretty normal in my experience. Prostate bruising is a definite thing. After being caged for a while and then released my shaft also ached when getting an erection.
We've never tried using a butt plug etc. I do hurt sometimes having sex. It seems to be an unsurprising effect of being in the cage most of the time.
I think it's normal. Certainly the aching shaft. I believe it comes from attempted erections inside the cage. Doesn't seem to be permanent. In term of the prostate, it all depends on what "noticable" is. Lot's of pain, bad, some discomfort, tiredness/achiness, pretty normal. If it is more on the some discomfort side of the scale, you'll learn to love it.
About a year into our cage use, when I’d be released after a longer lockup and I’d be allowed a full cum, it would sometimes be very unsatisfying at best and sometimes it would be uncomfortable / painful.
