There is a 5 mm pin going in from the front and coming out of the PA hole. This pin is fixed in the front an locks with a small padlock underneath. The cage is very lightweight and does not pull in the PA hole. There is also a recess or you could say different diameters for the penis head and the shaft which also holds it in place.

No I don’t think it will work. You have to convince your wife and maybe yourself about the PA. It is not that big issue
Just to pry a little, but what aspect of the penicap do you think wont work with the tetherspout? is the tip not firm enough?
The tip is firm but the lock is sitting underneath the casing. I am not sure how you would do the locking then. I suggest that you maybe ask the seller on “lustlock.com”. There you can also see some more pictures on how the locking is working.
Updating this thread in case captivated comes back online - the penicap works really well with a tetherspout. Took me 2 weeks to get the retainer to work properly but after that I have a ring through the spout for now then will move to a small lock.
