Yes, you need to prevent the erection from getting started. A shorter cage, which is close the correct girth when the member is shortened works the best. That cage looks too long to me. I am now in a cage which is less than 2 inches total length to the rear of the back ring. Restrictive, but a lot better to wear on a daily basis.

Also, be careful not to use too small of a rear ring around the package.
