I've been locked 24x7 for just over 5 years. I think I must be very fortunate in my anatomy: I found each of my first 3-4 cheaper cages to be quite comfortable for extended wear. Long-term comfort has been our goal since switching to custom devices, and I've been able to find it in two so far.

My stainless steel Watchful Mistress was comfortable enough to set a personal record of four weeks of continuous wear, but it wasn't perfect. I like to stay active and found stainless steel a bit bulky, especially for running activities. I also didn't have the measurements dialed in just right to completely avoid occasional testicle slippage.

Making the switch to titanium has eliminated the weight problem, and smaller measurements combined with an ergonomic flat ring seems to have eliminated the slippage so far (but fingers crossed). Today marks three weeks of continuous wear since my BA-05 arrived and, barring no slippage down the road, it's as close to perfect as I can imagine.

I can't tell if I'm wearing it or not. I can go for hours without noticing it's on. However, I feel naked when I don't have it on. I feel like I could go to the grocery store in just my cage, but I'm not going to try it.
