I tried urethral tube but when we close the cage I felt terrible and was almost going to black out.

Does anybody experienced that? Should I get used to it without cage before starting to use it?

I have never had that problem. How long is your tube and what diameter?

As others have said, stop using it and take a moment.

How long and how wide are both important questions to figure out what's going on. Also, is it metal or silicon?

The urethra needs careful, gentle stretching if you're not already into sounding. You need lots of lube, lots of patience, to insert anything in there.

Everything going in there needs to be sterile. Immaculately so. Otherwise you're asking for a UTI. If you're not regularly boiling or soaking in alcohol your tube then you should start.

That aside, to your actual question, based on your description it seems like the depth is the problem. Not the width.

The urethra is curved, takes a turn upwards to the prostate and then the bladder. If you're inserting something straight and metal that's considerably long there's a chance you're hitting the curve and putting pressure on the urethra in a direction it doesn't want to go rather than curving round with it. That could explain the pain.

Give us more details if you can.
