How long can you go without being emptied before possible health issues?

In my experience after 3 to 6 months, depending on circumstances, involuntary emissions/discharges occur on their own. You don't need to worry about emptying your balls. Mother nature will take care of things.

I think the "need" to milk a man in chastity is more a desire and/or wish that enhances D/s aspect of having one's penis locked. In other words some guys like the idea of being "required" to put something up their butt.

 

The generalized need, perhaps, as I said 'blue balls' with pain is quite different from everyday uncomfortable swelling of the pinched off trapped testicles
Besides, it gives an opportunity for fun discipline see you got them into that predicament!

On the topic of the necessity to drain one's balls, here are a couple of things that I've learned since we started this awesome game...

1. The only thing that your testes (balls, nuts) do is produce the sperm. They don't hold any ejaculate, so those who claim they feel full, and need draining are just experiencing a little wishful thinking.

2. The prostate, which makes prostatic fluid, just one component of come (along with seminal fluid from the seminal vesicle, and sperm from the testes), is self maintaining. Some guys have wet dreams to release fluid from the prostate. I've never had one. After a couple of weeks of denial, I notice that I often expel a slug of prostatic fluid when I urinate. It's the excess fluid that the prostate needs to get rid of.

So, if your balls are all swollen and tight, and you're afraid if your keyholder doesn't let you cum, that they're going to explode, have no fear, it's all in your head.
