I have a HOD metal cage with a 45 mm circular base ring. It fits very well when soft and there is no room left in the cage. The base ring when I'm soft sits about 5 mm from my body but when I try and have erections (night ones) the cage seems to slide down as I grow which results in a gap of about 25 mm from the base ring to my body.
Is this normal or is my device not fitting me well?
How do I keep the bar ring next to my body so to restrict my penis, as if the cage moves when I try and grow, to me it defeats the point of the cage.

This happens, it is quite normal. Some cages have loops to attach a belt to reduce or prevent this. HOD have some with an optional belt.

I might look at the belts to keep it closer.
    Cheers

I have one of those elastic belts and works well. The cage stays in place and guess say lifts it and out

Can I ask where you bought it from?

If you want to try a waistband with your existing cage you could use some paracord. Here is the sliding knot I use to make it adjustable (more pics in my gallery):
