So due to a permanent medical issue one testicle is significantly larger than the properly sized base ring, like 13mm or so larger.

Hinged rings seem to be ill advised in general, and multi part ones are typically plastic and seem like assembling a jigsaw puzzle in the dark.

    Any thoughts would be appreciated.

That shouldn’t matter really, cause you put one ball through at a time.

Go for the smaller ring size that fits and holds the smaller ball.. my left ball is bigger but fortunately lower than the right, so the ring size prevents just right ball from slipping out… which is mostly the case of ball(s) escaping. Time amd age will tell if both balls should loosen down more for better comfort.. too low-hanging balls will create another set of problems…

Unfortunately, the issue is the lower testicle is unable to fit through a properly sized base ring by quite a bit.

if you one testicle can’t fit through a 1 3/4 inch diameter or 45mm w/e you wanna call it… I would suggest getting a hinged ring that you don’t need to push your balls through.
