I think it is a combination of things. It mostly takes some level of interaction, even just talking about it with another person, for me to become physically aroused at this point, but looking at an image or video clip does cause some emotional excitement. I suspect that chemicals are being released in the brain in an attempt to compel me to "go all the way". Of course completion never happens, so you are just left with some excitement which slowly drops away. If I become physically aroused, that is less pleasant and more frustrating due to the size of the cage. After a prolonged period it becomes very frustrating to not be able to have an erection.
