Search "bullpen" underwear, or look for anything with a pouch. You want support and breathability primarily. If you are into ladies garments, I've read that panties are where it's at, but don't have any firsthand experience to share on that front.
some of mine do have the pouch and they feel the best and dont smash the balls. lets them hang better

Panties are best for me as, since you pee sitting down anyway, you don't need the penis access of men's underwear! Since I play sports daily, I wear "boyshort" style panties which have more room in the gusset area (between the footholes). This provides room and support for the cage without permitting "ball popping" out of the panties. And, of course, the silky feeling of wearing panties is exquisite!
Depends Realfit. No, really. Try it. The padding helps to make the little pinches through the bars not a problem, and absorbs all the drips of urine and "tears" that happen.
I like boxers as boxer briefs made cage too pronounced of bulge for liking. When I was wearing my heavier cage I used an Amazon elastic strap for support. Did a better job and didn't cause the obvious bulge issue.
I find I get the best support and comfort from those that have a pouch, and the fabric has some stretchy latex or elastine in it. Personally I much prefer a cotton mix as it's more absorbant and less likely to cause skin problems from humidity than a purely man-made fibre which isn't so absorbant, but these days some man made fibres are just as good. I'm not sure which are best so I tend to go for a 95% cotton, 5% elastine mix which is fairly common and works well for me.

I’ve found good tight briefs/thongs/jocks are best, they hold everything in place and reduce movement that in turn reduces irritation and discomfort.
Like others in this thread, I’m pantied along with my cage, and the two are inextricably linked. Hi-cuts tend to work best for me as they have ample room for everything without being granny panties.
It depends on what you wear I had some tight underwear on today and nothing moved and was so uncomfortable
