Not uncommon to have ring to rotate slightly.

 

It looks okay to me. You should be able to rotate the cage so it lines up better if you want. Just be careful about it.

The biggest thing you need to worry about is cutting off blood flow. If you can get a finger in the ring between it and your balls you should be fine.

Watch out for any sort of pain in the balls, discoloring (turning red / purple / blue), or the balls get very cold. Getting one of these is not good, getting more than one is very bad and you need to remove immediately.

Note - they will generally get cooler, because they're away from your body and that's what happens.

One thing you're likely going to get is "ball burn", where there's an abrasion style burn happening under your balls in contact with the ring. I recommend a little bit of Aquaphor to help lubricate, but there are tons of advice on what to use on this site.

It might be a good idea to just go for a few hours the first time until bed time. Maybe not go for overnight the first time... Slowly seems to be the watchword.

Do you have any curve to your shaft when your hard? Or are you perfectly straight out when hard? if you have any curve at all, and youve gotten an erection while hard, the cage will spin. My HT v2 did that everytime it wore it. Make sure you keep some short of lube under the ring and inside the tube. I used to use baby oil gel. Apply it with a q-tip. Now, I use coconut oil and it works better then the baby oil gel. WIth the HT make sure you take it off at least weekly to clean. No matter how well you "think" you cleaned it, its not clean enough as an open cage. I loved the HT though. I wore it for 8 months only removal was during a cleaning which I was handcuffed behind my back and the wife removed it, cleaned me and the cage, and then put it back on.
