Surgilube is the most commonly used one. Like the name implies, it's a medical lube that's sterile, water-based, and bacteriostatic. It's also relatively inexpensive. If your insert is metal, than a high quality silicone lube like Eros Pur is a good choice.

It may depend on the quality of device you are using. I have the Looker 01 which is a finely crafted item and only used lube once, on the first short try. After some research I realised that just being wet was sufficient lubrication. So I ensured it was clean and very wet prior to it going in. Straight out of the water and in, and I never had any problems. The urethra is well used to water and I found it worked well just using that. Obviously the insert has to be made such that it can cause no damage to the very delicate passage. Some are happy with DHgate ones but be careful.
Likely many factors here, length, material and experience. It shouldn't hurt, if your body accepts it without lube great, if not surgilube is your best option. You may find that after you've gotten your body used to the feeling of inserting something into your urethra you no longer need the lube.

All said and done, just do be aware that some Lubes will dry out after a time. I've known of three cases now where this has happened all three resulted in tearing of the lining of the urethra as the insert was withdrawn. I have a cage similar to the you showed. I haven't had any problems with it. I use a silicone based lube. I do clean it before and after each use. As with any insert you should only leave it for 5-6 hrs max at a time. There is chance for stuff to get up the tube and start an infection.

I based my option on guys that have worn them as well as advise from a urologist. Wasn't trying to to upset anyone it is just my option based on my research. Sorry to cause mixed felling on this subject.
I think you will find, if you did consult a urologist that he would have had no personal experience of long term insert wearing as well as having no records of any patients who have ever worn inserts in chastity for extended periods. Furthermore, any professional urologist worth his salt would advise you not to attempt such a thing under any circumstances.
