I can see a couple of ways that could be used... - The long bar has a ring at the tip away from the base ring, so maybe it's intended to be used with a large captive bead ring in a PA. or - the long bar comes off by undoing the locking screw from the base ring, the bar is inserted into the tip of the penis and out of the PA, and then fixed back to the base ring with the locking screw.
Hah - I laughed so hard, too! The device was totally out of specification, and it was useless when I received it. I was very disappointed when it didn't fit at all, especially considering my efforts to communicate the details of my order. It took three months to arrive, then another month to get it back with "adjustments." Afterward, I still had to heat and hammer the device into the appropriate shape. Communication, production speed, and order accuracy were poor. I went out of my way to articulate my needs expressly, and still struggled to get something i could use. Ultimately, I exclusively use the device during the day, because it has the potential to stress my piercing overnight. Other than that, it's very comfortable and unobtrusive throughout the day -- and I'm very active outdoors. I don't even notice it during daily life, except during periods of arousal. In it's final form, the device does not resemble the one that is currently depicted on the website.
