For anyone that uses a cage that is not ball trap but attaches to a frenum piercing, what's the best gage comfort wise. Thanks in advance for any help!

It depends if you are circumcised. The size of the foreskin matters for selection. I am not circumcised and find it hard to stow a frenum based anti-pull-out device together with the foreskin.
when I got mine (just below the head) they used an 8ga. I got a device from evotion, but it still has the ball trap. I went a little roomier on the base ring, since I no longer had to worry about pull-out. I went for the combo, because I was afraid that without the trap, I would be able to stroke the lower part of the shaft. Because of this I haven't worried about the Guage. I would say that without the ball trap you need to be very careful how the cage is designed to keep pressure off the piercing. It needs to be snug enough that the weight of the cage doesn't hang on the piercing when soft and located properly so that when it tries to get hard, it doesn't try to push the cage off and pull on the piercing. Its thin skin down there. On a side note, when my wife does want to use her cock, she enjoys the piercing, especially in doggy.
