Well as we see in different languages things are described different.
In German you will find the Penisfraktur (fractured penis) even in a medical textbook. It is commonly used by everybody - even medics.

It is a injury of the tissue of the erectile tissue. The reason is a bowing momentum during sexual practises, when the penis is hard. The patients can hear a crack.
You get an hematoma and swollen.
Afterwards you can feel a dent in the tissue.

That was my thought after he discribed his problem.
Full erected in the cage, the erection is bent down and an additional momentum leads to the mentioned injury.

If I am wearing bigger cages the weenie erects into the cage and pushes the cage a bit away. If you wear a small cage the penis makes a kink. Krrrrk.
