I have a cobra style stainless steel cage. It is a pretty good cage.
Comfortable, there is a little pinching if you hit it a certainl way, but nothing crazy.
The problem with this style cage is that pulling out is ridiculously easy.
I’d even just slipped out of a couple of times on a really sweaty day.
Any cage can be escaped without a piercing or urethral system, but this one is too easy.

edit, I will say that I seem to have significantly less nocturnal erection in this cage than any other.
Even my newest cage, a red chili, fits like a glove but I still get nocturnal erections (although it is mostly inside the body and not extremely uncomfortable like my MM QK).
But in that cobra style cage I get almost nothing, ever. Not even during waking hours.
But then I suppose if I wanted an erection, I could just pull it out .

I have a tetherspout and the lock from badass workshop. I would highly recommend it. I don't really consider it a uretheral device even if it technically is. I don't notice any sensation in the urethra from it. The only sensation I have is the head of the penis very slightly when I get hard of if I pull it hard enough.

 

I have no experience with any cobra style device, but if your key holder is wanting any device to be escapable without a piercing, then a spiked device is your only option I feel.

 

You might be able to make a “headlock” from some moldable plastic that fits inside of the device. Lookup instamorph.

 

Do you have the x-ring? It takes away the gap. Also I would suggest a smaller cage
