I saw this on Etsy too. I just bought 2 new cages and this is one I was looking at as well. I didn’t buy it but I’d be interested to hear how it works IRL.

 

I've had a real problem with sizing rings; large enough to be comfortable, and a testicle will spontaneously escape. Tight enough to stay on, friction burns to the point of skin damage, even with copious lubrication. Lately, I've been experimenting with the Chinese "anti-off spike ring" with spikes ground down to nubs that just intrude into the diameter of the penis tube. This seems very promising.

The cage in the OP seems conceptually similar, so I'd suggest going up a ring size or two and give it a whirl (for science!). I hope the scrotal bar makes it's way to more manufacturers.

I've contemplated something similar for my devices or any future ones I might make and it certainly is an interesting prospect.
The problem is when you increase the complexity of anything, you just introduce more places where it can break and, in the case of cages, more places to pinch I would suspect.
I think there are better ways to go about this idea but its good that people are exploring more options (especially the chinese knockoff people which are moving past being complete leeches)

 

I tried this one -the space behind the scrotum bar is very small, could not get it to fasten. Reminded me why I went to custom devices....

 

I've seen them and going to buy 1 just for something different
