It happens from time to time. I've used polymorph plastic instead - somewhat safer to remove when you finally have to. "Permanent" style chastity isn't for everybody.
Polymorph is great fun to use, and because it's relatively easy to cut or drill, as Giles says, a lot safer than bonded metal when you eventually need to get free.
