It seems a very permanent situation, re-routing your urethra behind your balls and keep the penis an a curve downwards. Who would do such a surgery without questions asked? I would imagine this to be painful...
I have heard of this and many more extreme measures. I'm not too sure what the ring is supposed to do though.

Be careful with a re-route. If the urine exits behind the balls you run the risk of more Infections if urine isn’t passing (or pissing) fully down the urethra

yes, there is a self-contained ring that goes through the urethra of the penis and therefore a reroute is necessary...
A new hole is made in the urethra behind the balls. A ring is then threaded into the end of the penis, and exits through the new hole. This makes it impossible to use the penis to penetrate anything. An erection is forced to follow the curve of the ring, downwards.
In theory, it is no worse than for a woman. It is apparently a challenging modification to make. Really requires a surgeon's skills, because of the vasculature in that area.
This topic is dangerously close to my obsession with genital modifications that led to me being kept chaste in the first place. This would be something I would have in an instant if my Wife asked to. I don’t know how practical it is though given you have a metal ring in situ 24/7. Sitting down and the penisnjutting forwards must be an issue? The body mod or piercing you are talking about here is a very old one. It was used by Australian Aborigines as a method of birth control. Possibly others too. Basically they made a hole (a subincision) and kept it open till it healed as a hole. When they wanted kids or to piss normally they just put a finger over the hole as they ejaculated. Now where ypu'd ge that done I don't know but if you speak to an experienced Body Mod practitioner they may either point you in the right direction or be willing to do it. I don't think I'd go with someone that has not done it before several times to people who are willing to talk to you about the total of their experienc of the the process, the healing and the reality of post healing. I dare say it would be repairable but not cheap.
